COVID-19 Patient Safety Requirements

  • Dr. Boudreaux and the team at Maison Optique Vision Center care about the well being of our patients and our staff. The following guidelines have been put in place to reduce the exposure of COVID-19. We sincerely appreciate your understanding as we re-open our office and continue to provide exceptional care and service to our patients.
  • Prior to your Visit:
    1. Please sign and complete all necessary forms before arriving for your appointment.
    2. You must provide all Medical Insurance or Vision Plan information that you plan to use so it can be verified and authorized prior to your visit.
    3. Provide the reason for your visit along w/ other pertinent information by:
      • Submit information through our patient portal. Log in info can be sent to you via text or email
      • One of our technicians can contact you by phone to take your information.
    4. Please bring a mask to wear during your visit.
  • What to do and expect when you arrive for your Visit:
    1. Please enter the office alone for your visit. We will be allowing a maximum or 3 patients at one time in the office for proper social distancing. The following exceptions are:
      • The parent or guardian of a child.
      • The caregiver for and elderly or disabled patient.
    2. Please wear a mask for your office visit.
    3. Our doors will be locked. Call our office when you arrive and we will escort you in.
    4. Your temperature will be taken with a non-contact thermal thermometer upon entering the office.

Despite being temporarily closed for routine eyecare, we continue to provide helpful services for our patients, such as emergency eyeglass repairs/warranties and contact lens orders.
